<p>The<a href="http://www.gatesfoundation.org/Pages/home.aspx" target="_blank"> Bill &amp; Melinda Gates Foundation</a> recently renewed its commitment to the Global Fund to Fight AIDS, Tuberculosis and Malaria, giving $750 million through a promissory note. This innovative funding method gives the Global Fund the flexibility and authority to distribute funds efficiently based on immediate needs, leading to greater impact.</p>

Since that time, over 65 million people have been infected with the human immunodeficiency virus (HIV) and over 30 million have died. Currently, 34 million people are living with HIV and 2.7 million people were newly infected in 2010 alone. At the same time, great advancement and progress have occurred to address the epidemic. Global programs including the <a href="http://www.pepfar.gov/">U.S. President&#8217;s Emergency Plan for AIDS Relief (PEPFAR)</a> and the <a href="http://www.theglobalfund.org/en/">Global Fund to Fight AIDS, Tuberculosis and Malaria</a>,  the <a href="http://www.gatesfoundation.org/Pages/home.aspx">Bill and Melinda Gates Foundation</a>, the <a href="http://www.clintonfoundation.org/">William J. Clinton Foundation</a> and others are working to provide life-saving antiretrovirals, develop new methods of prevention and improve the quality of life of those living with HIV/AIDS around the globe.</p>

			<content:encoded><![CDATA[<p>With <a href="http://www.worldaidsday.org/">World AIDS Day</a> coming up on December 1st, I had the opportunity to join in a call with <a href="http://www.gatesfoundation.org/leadership/Pages/stefano-bertozzi.aspx">Stefano Bertozzi</a> <span><a href="http://www.gatesfoundation.org/hivaids/Pages/default.aspx">Director of HIV and TB at the Bill &amp; Melinda Gates Foundation</a>. </span></p>
<p>World AIDS Day is a time to renew our commitment to the 34 million people living with HIV/AIDS worldwide and the recent developments revolving around HIV, including several scientific breakthroughs in HIV prevention research and <a href="http://www.state.gov/secretary/rm/2011/11/176810.htm" target="_blank">Secretary Clinton’s recent remarks</a> declaring that it is possible to reach an AIDS-free generation, make this an especially exciting time to reflect on the epidemic.</p>
<p>According to Dr. Bertozzi, the strategy of the <a href="http://www.gatesfoundation.org/Pages/home.aspx">Bill &amp; Melinda Gates Foundation</a> includes both decreasing new infections and improving quality of life of those currently infected. A large focus of this strategy is on the delivery of current strategies in large part through the <a href="http://www.theglobalfund.org/en/">Global Fund</a> as well as smaller national programs.</p>
<p>Despite the proven effectiveness of existing prevention approaches, there are limitations. For example, many women are not in a position to insist that their partners wear condoms and daily adherence to prophylactic treatments is difficult. The development of a vaccine and other new prevention options is critical for successfully fighting the epidemic, especially in developing countries. The largest investment in newer technologies is currently in trying to develop a <strong><a href="http://www.hvtn.org/">HIV vaccine</a></strong>. As such, a focus is on the success of the <a href="http://articles.cnn.com/2009-09-24/health/hiv.vaccine_1_hiv-vaccine-vaccine-trials-aids-vaccine-conference?_s=PM:HEALTH">RV144 HIV vaccine trial</a>, which recently showed encouraging results in Thailand and advancing that to the next generation of the vaccine with hopes of increased efficacy.</p>
<p>They are also investing heavily on products that can be used by individuals to protect themselves either topically in the vagina or systemically. While daily use of vaginal <strong>microbicides </strong>has shown mixed results, many reasons may contribute to this, especially lack of adherence which is a common problem with daily use products. To address these challenges, they are working on products which are less dependent on adherence such as a <a href="http://www.globalhealthtv.com/news/v/new_study_into_vaginal_ring_looks_to_hiv_aids_prevention/to/latest_news/">vaginal ring</a> which slowly releases the antiretroviral compound dapivirine and can be left in for up to a month.</p>
<p>Another strategy is systemic daily antiretroviral prophylaxis which has also had mixed results and faces problems with adherence, but looks promising. They are focusing on injectables which can be injected every 1 to 3 months, decreasing the need for adherance. The Gates Foundation is currently supporting clinical trials to evaluate the effectiveness of <strong><a href="http://www.cdc.gov/hiv/prep/resources/qa/index.htm">Pre-exposure prophylaxis</a></strong> (PrEP) such as this for HIV prevention.</p>

<p>Additional efforts are on enhancing the delivery of <strong><a href="http://www.newscientist.com/article/dn17312-bill-gates-helps-fund-mass-circumcision-programme.html">male circumcision</a></strong> and scaling up these programs with fewer resources. Despite three clinical trials demonstrating the protective benefit of the procedure, donors and countries have been slow to invest in voluntary male circumcision for HIV prevention. The foundation is investing in advocacy efforts to encourage more rapid scale-up of male circumcision for HIV prevention and funding research on new technologies and methods for performing male circumcision safely and less expensively. This strategy is so cost effective that it costs more money to NOT implement such programs due to the future treatment savings.</p>
<p>The Gates Foundation is also working towards improving the delivery and effectiveness of current programs. With decreasing funding in a faltering global economy, scale up of treatment has continued at the same pace by improving efficiency of programs. Fortunately this has been happening in HIV treatment. Continuing efforts to reduce the cost of drugs, design and implement more efficient delivery systems and further task shifting and sharing among health providers is needed without compromising care and can even lead to improved quality of care.</p>

			<content:encoded><![CDATA[<p><em>As promised, today we bring you another commentary inspired by <a href="http://www.nytimes.com/2011/11/09/health/policy/hillary-rodham-clinton-aims-for-aids-free-generation.html">Secretary Clinton&#8217;s recent speech</a> calling for an AIDS free generation by Matthew Basilico, Nworah Ayogu, and Arjun Suri of <a href="http://hms.harvard.edu/hms/home.asp">Harvard Medical School</a>. </em></p>
<p>As students immersed in the study of biological sciences, and as future physicians anxious to provide care and improve lives, we can be frustrated when public health policy seems based on interests neither scientific nor beneficent. United States programs that fight global AIDS have accomplished tremendous good over the past eight years; however, recent stewardship by President Obama has been disappointing both scientifically and morally. Last fall, many of us at Harvard Medical School protested President Obama because we believed his failure to keep his promises to scale-up the fight against HIV/AIDS was proof that he was ignoring the science and neglecting his ethical obligation to save millions of lives around the world. However, on Tuesday, Secretary of State Hillary Clinton delivered <a href="http://www.youtube.com/watch?v=ClXECbOv1mY&amp;feature=related">an inspiring address</a> that is at once evidence-based and morally laudable.</p>
<p>When we began our protests last fall, there was a growing body of research indicating that treating HIV also prevents its spread. Evidence from a localized study South Africa indicated that when people with HIV received antiretroviral treatment (ART), their partners were 92% less likely to contract the virus. [1] Initial modeling used this information to theorize that, with universal access to treatment, infections could plummet within 10 years. [2] With this growing evidence (as well as other studies), the phrase “treatment as prevention”—long used by health professionals working at the front lines of AIDS care—resonated in scientific communities. Not only does treatment save lives, but these recent studies show that treatment could dramatically reduce new infections, slowing or even halting the epidemic in the future. It was not until this year, however, that this could be said with such certainty. A multi-site, large-N, randomized control trial showed that ART reduces new infections by 96%. [3]</p>
<p>We have also been encouraged by the growing literature showing that investments in HIV treatment programs improve health systems and eases delivery of other life-saving interventions.[4,5]  Dr. Paul Farmer, co-founder of <a href="http://www.pih.org/">Partners In Health</a>, described HIV treatment as the <a href="http://www.youtube.com/watch?v=72Qz0WsQOAo">“battle horse”</a> to drive the necessary expansion in infrastructure and political will for addressing other global health priorities.</p>
<p>The evidence is clear: now, more than ever, we know that investing in AIDS treatment will save millions of lives and reduce new infections. Yet in the first years of his administration, President Obama fell dreadfully short on campaign promises to significantly improve resources for AIDS funding.  Many fellow classmates from Harvard Medical School, as well as from colleges and medical schools across the east coast, joined in protests encouraging the administration to uphold its promises.  It is impossible to forget the numbers—a year of AIDS treatment costs less than $100, and the treatment program makes up far less than 0.2% of the federal budget.  Yet while hundreds of billions of dollars went to bank bailouts, no new money was found for the meager $1 billion dollar annual rise that was promised during the election cycle.</p>
<p>Secretary Clinton’s speech on November 8<sup>th</sup>, therefore, was encouraging.  She declared that for the first time, it will be United States government policy to create “an AIDS-free generation.”  She emphasized prioritizing high impact interventions—prevention of mother-to-child transmission, circumcision, and treatment.  And she lauded the vital role of the Global Fund to Fight AIDS, Tuberculosis and Malaria, which has been an innovative and transparent engine for delivering resources where they are needed most.  Secretary Clinton’s speech had much of what is needed, except for the numbers.</p>
<p>To transform the Secretary’s vision into a reality, we will need bold treatment targets and solid commitments to increase funding.  Six million patients on treatment by the end of 2013 would be an appropriate goal for the United States government, which is on track to have four million on treatment by the end of this year.  Congress and the administration have responsibility for the 2012 budget, while the Super Committee debates 2013 and beyond.  We eagerly await President Obama’s address on <a href="http://www.worldaidsday.org/">World AIDS Day</a> (December 1<sup>st</sup>).  Lawmakers have the opportunity to transform one of the greatest human scourges of our generation, or to be held accountable for inaction by patients, activists and history books.  As medical students, we have the ability to hold lawmakers accountable by calling and writing our elected officials, and communicating publically the cost of inaction.</p>

<p>Yesterday, Secretary of State Hillary Rodham Clinton <a href="http://www.state.gov/secretary/rm/2011/11/176810.htm" target="_hplink">declared</a> the U.S. government&#8217;s intent to create an &#8220;AIDS-free generation.&#8221; Secretary Clinton <a href="http://www.state.gov/secretary/rm/2011/11/176810.htm" target="_hplink">outlined</a> a bold plan to reduce new HIV-infections, globally, including the eradication of pediatric HIV by 2015. This new strategy builds upon the success of the President&#8217;s Emergency Plan for AIDS Relief (<a href="http://www.pepfar.gov/" target="_hplink">PEPFAR</a>), the U.S. program that addresses HIV/AIDS in resource-limited settings.</p>
<p>A notable feature of Secretary Clinton&#8217;s &#8220;AIDS-free generation&#8221; initiative is to strengthen healthcare systems in sub-Saharan Africa. Clinton <a href="http://www.state.gov/secretary/rm/2011/11/176810.htm" target="_hplink">stated</a>:</p>
<blockquote><p>&#8220;We know we can&#8217;t create an AIDS-free generation by dictating solutions from Washington. Our in-country partners &#8212; including governments, NGOs, and faith-based organizations &#8212; need to own and lead their nation&#8217;s response. So we are working with ministries of health and local organizations to strengthen their health systems so they can take on an even broader range of health problems.&#8221;</p></blockquote>
<p>Strengthening African healthcare systems is a view echoed by many eminent voices in the global health community. Last year, the Institute of Medicine (IOM) of the National Academy of Sciences authored a report entitled: &#8220;<a href="http://www.iom.edu/Reports/2010/Preparing-for-the-Future-of-HIVAIDS-in-Africa-A-Shared-Responsibility.aspx" target="_hplink">Preparing for the Future of HIV/AIDS in Africa: A Shared Responsibility</a>.&#8221; The IOM report recommended the urgent need to increase African healthcare workforce capacity to address the HIV epidemic.</p>
<p>I offer Secretary Clinton a solution to assist African healthcare workforces and ensure the success of the &#8220;AIDS-free generation&#8221; initiative. Last year, in an <a href="http://www.nejm.org/doi/full/10.1056/NEJMp1006501" target="_hplink">editorial</a> in <em>The New England Journal of Medicine</em>, Vanessa Kerry, Sara Auld, and Paul Farmer reintroduced the idea of enacting a <a href="http://www.globalhealthservicecorps.org/" target="_hplink">Global Health Service Corp </a>(GHSC). The GHSC, compromised of U.S. healthcare professionals, would provide medical education and technical assistance to enhance the healthcare workforces in low-income countries. The GHSC&#8217;s <a href="http://www.nejm.org/doi/full/10.1056/NEJMp1006501" target="_hplink">goal </a>would &#8220;go beyond that of filling a human resource void to focus on infrastructure development, knowledge transfer, and capacity building.&#8221; The GHSC could also offer partial student loan forgiveness for U.S. corps members who engage in service abroad for a specified time period similar to the loan forgiveness offered by the National Health Service Corp.</p>
<p>To address the African healthcare workforce shortage, I encourage Secretary Clinton to adopt the <a href="http://www.globalhealthservicecorps.org/index.php/ghsc-2/a-proposal-for-a-ghsc/" target="_hplink">principles</a> of the GHSC. The success of the &#8220;AIDS-free generation&#8221; initiative depends on the availability of skilled healthcare workers in African resource limited settings. Additionally, the eventual transition from a U.S. to African led HIV/AIDS response requires the U.S. to teach and train healthcare personnel in recipient countries through collaborative partnerships that eventually lead to African ownership of their domestic healthcare needs.</p>
<p>Some may argue that enacting the GHSC, especially in the era of U.S. government austerity measures, is not prudent. However, the funding for the GHSC already exists. In addition to HIV/AIDS prevention and treatment, PEPFAR&#8217;s congressional mandate requires the program to &#8220;strengthen partner government [healthcare] capacity to lead the response to this epidemic and other health demands.&#8221; Last year alone, PEPFAR committed over $734 million in healthcare capacity building initiatives.</p>
<p>Global health is the moral litmus test of our time. As Secretary Clinton asserted: &#8220;An AIDS-free generation would be one of the greatest gifts the United States could give to our collective future.&#8221; The U.S. should enact the GHSC to ensure the success and sustainability of the &#8220;AIDS-free generation&#8221; initiative.</p>

			<content:encoded><![CDATA[<p>With the HIV/AIDS epidemic marking its 30th year, over one million Americans are living with HIV today, yet 1 in 5 of those infected doesn’t know his/her status according to the <a href="http://www.cdc.gov/">CDC</a>.  Further, one-third are diagnosed at a late stage of infection and go on to develop AIDS within one year. The <a rel="nofollow" href="http://mailview.bulletinhealthcare.com/mailview.aspx?m=2011062401ama&amp;r=1771830-5d5e&amp;l=037-4e9&amp;t=c" target="_blank"><span style="text-decoration: underline;">Huffington Post</span></a> (6/24, Pearson) reports that almost 40% of Americans age 18 and older have been tested for HIV at some point in their lives, increased from 32.1% in 2000 but still dramatically less than the CDC&#8217;s recommendation that everyone between the age of 13 and 64 undergo routine HIV screening. Yet much progress had been made since the epidemic came to light in 1981. A recent <a href="http://www.nytimes.com/2011/05/31/health/31aids.html?_r=1&amp;partner=rss&amp;emc=rss">New York Times article</a> looks at the progress and research that has occurred in the past 30 years.</p>
<p>The <a href="http://kff.org/">Kaiser Family Foundation</a> recently completed a <a href="http://www.kff.org/kaiserpolls/8186.cfm">survey</a> of 2,583 American adults ages 18 and older to analyze public opinion about HIV/AIDS.</p>
<p><a href="http://www.kff.org/kaiserpolls/upload/8186.pdf">According to the survey</a>:</p>
<ul>
<li>There is a decreasing sense of national urgency and visibility of HIV/AIDS.</li>
<li>African Americans, particularly young men, expressed higher levels of concern about HIV than Caucasians.</li>
<li>After almost a decade of decline, an increased number of Americans reported that they were personally  &#8220;very concerned&#8221; about becoming infected.</li>
<li>More than 50% of Americans support increased funding for HIV/AIDS, with  fewer than one in ten reporting that they feel that the federal government spends too much on HIV/AIDS.</li>

<p>During a recent <a href="http://globalhealth.duke.edu/about-the-institute/">Duke Global Health Institute</a> talk as part of <a href="http://globalhealth.duke.edu/news-events/global-health-news-at-duke/20-student-groups-gear-up-for-global-health-week-2011">Global Health Week 2011</a>, Ugandan physician and HIV/AIDS pioneer <a href="http://www.who.int/world-health-day/previous/2006/doctors/uga/en/index.html">Peter Mugyenyi</a> gave a talk on the unresolved AIDS crisis in Africa. In his talk, Mugyenyi reiterated that, although progress has been made in the fight against HIV/AIDS, the epidemic is far from over. He emphasized that the implementation of the<a href="http://www.pepfar.gov/"> US President&#8217;s Emergency Plan for AIDS Relief (PEPFAR)</a> and the <a href="http://www.theglobalfund.org/en/">Global Fund to Fight AIDS, Tuberculosis and Malaria</a> have played large roles in saving the lives of millions around the globe. For example, Mugyenyi directs the <a href="http://www.jcrc.org.ug/">Joint Clinical Research Center in Kampala</a>, which receives funding from these organizations to bring life-saving antiretroviral therapy to hundreds of thousands of Ugandans. In the following video of Mugyenyi&#8217;s talk at Duke, he shares his thoughts on the role of universities in the fight against HIV/AIDS and his hopes and concerns for the future.</p>

			<content:encoded><![CDATA[<p>Today, December 1st, is <a href="http://www.worldaidsday.org/">World AIDS Day</a>, a reminder of the tragedy of the epidemic as well as the progress which is occurring. According to <a href="http://www.unaids.org">UNAIDS</a>, <a href="http://www.unaids.org/documents/20101123_FS_Global_em_en.pdf">over 33 million people in the world are living with HIV, including 1.5 million in North America</a>. While great injustices exist in access to care for many of the millions living with HIV/AIDS around the globe, human rights efforts seem to be increasingly integrated into national AIDS strategies with 89% of countries explicitly addressing human rights in their AIDS strategies and 91% implementing programs to reduce discrimination and stigma related to HIV/AIDS. With regards to funding targeting the epidemic, while an estimated US $15.9 billion was spent towards HIV/AIDS efforts in 2009, declining international investments are expected to significantly impact many around the globe, especially those in low-income countries which rely heavily on international funding of their HIV/AIDS programs.</p>
<p><a href="http://www.unaids.org/documents/20101123_2010_HIV_Prevalence_Map_em.pdf"><img class="aligncenter" title="HIV Prevalence Map" src="http://www.unaids.org/globalreport/App_Themes/img/prevalence_map.png" alt="" width="605" height="276" /></a></p>
<p>The <a href="http://www.unaids.org/documents/20101123_GlobalReport_em.pdf">2010 UNAIDS report</a> highlights updated information regarding the epidemic, including the growing number of people practicing safe sex, and gaining access to antiretrovirals in many parts of the world. Offering hope that progress is being made to quell the epidemic, UNAIDS reports that at least 56 countries have either stablized or achieved significant declines in rates of new HIV infections, with new HIV infections declining almost 20% in the last five years. At the same time that rates of new infections are declining overall worldwide, the number of new infections are increasing in areas like Eastern Europe and Central Asia, in large part due to injection drug use.<br />

<p><a href="http://www.huffingtonpost.com/anand-reddi/post_1016_b_752554.html"><img class="alignright" style="margin-left: 10px; margin-right: 10px;" title="Huffington Post Logo" src="http://www.sfnblog.com/HuffingtonPost-Logo.jpg" alt="" width="228" height="82" /></a>Yesterday, the international donor community pledged $11.7 billion over the next three years to fund<a href="http://www.theglobalfund.org/en" target="_hplink"> The Global Fund to Fight HIV/AIDS, Tuberculosis, and Malaria</a>. The fund had hoped to raise $20 billion. This shortfall in necessary funding will put at risk the 2015 goal to: eliminate HIV mother-to-child transmission worldwide, prevent the spread of multidrug resistant TB, and eradicate malaria as a public health issue.</p>
<p>A pressing question amongst global health advocates is how to finance global health in the midst of this global economic recession?</p>
<p>One solution proposed is the enactment of a financial speculation tax on the currency transactions market. The proceeds raised could fund global health initiatives such as the Global Fund and the <a href="http://www.pepfar.gov/" target="_hplink">President&#8217;s Emergency Plan for AIDS Relief (PEPFAR)</a>.</p>
<p><span id="more-1216"></span>Sixty countries including France, Germany, Japan, Spain and the United Kingdom are publicly<a href="http://www.halifaxinitiative.org/content/policy-brief-ftt-idea-whose-time-has-come-april-2010" target="_hplink">supporting</a> an international financial speculation tax on the $4 trillion dollar currency transactions market. In the United States, Congressman <a href="http://www.stark.house.gov/" target="_hplink">Pete Stark</a> (D-CA), introduced <a href="http://thomas.loc.gov/cgi-bin/bdquery/z?d111:h.r.5783:" target="_hplink">H.R. 5783</a>, the Investing in Our Future Act. Representative Stark <a href="http://www.huffingtonpost.com/pete-stark/currency-tax----a-way-to_b_652884.html" target="_hplink">proposes</a> &#8220;a small tax &#8212; of five thousandths of one percent, or 0.005% &#8212; on currency transactions. Due to the extreme speculation that takes place, it would raise [at least] $28 billion a year and reduce currency speculation by 14 percent.&#8221;</p>
<p>Some may argue, as fiscal conservatives surely will, a tax on banks is ill advised at this time given the anemic state of our economy. However, David Stockman, director of the Office of Management and Budget under President Reagan, <a href="http://www.nytimes.com/2010/01/20/opinion/20stockman.html?_r=2" target="_hplink">writes</a> in the <em>New York Times</em> that &#8220;while supply-side catechism insists that lower taxes are a growth tonic, the theory also argues that if you want less of something, tax it more. The economy desperately needs less of our bloated, unproductive and increasingly parasitic banking system.&#8221; A proposed financial speculation tax &#8212; a financial &#8220;pinprick&#8221; for international banks &#8212; would be an important stream of revenue to fund domestic and global development initiatives as well as reduce the reckless financial speculation practices of international banks.</p>
<p>Global health is the moral litmus of our time. As Nelson Mandela once said &#8220;Sometimes it falls upon a generation to be great.&#8221; Fifty years from now, will we want to remember that we bailed-out AIG, Citibank, and Goldman Sachs? Or will we want to remember that we committed the necessary funds to end the scourge of AIDS, TB, and Malaria?</p>
<p>I urge President Obama and the United States Congress to support Congressman Stark&#8217;s <a href="http://www.stark.house.gov/index.php?option=com_content&amp;task=view&amp;id=2008&amp;Itemid=62" target="_hplink">Investing in Our Future Act</a>. Together we can finance global health and protect Wall Street from the dangerous speculation practices that contribute to global economic instability.</p>

<p style="text-align: justify;">This comprehensive and integrated approach is what the President&#8217;s Global Health Initiative is all about. It recognizes that we can&#8217;t treat our way out of the HIV-AIDS epidemic. The key to ending it is to reduce the number of those who become HIV-positive in the long-term &#8211; and that takes improving their overall health and the health systems around them. After all, patients don&#8217;t come to doctors with one disease or condition, and our response shouldn&#8217;t focus on one as well.</p>

<p>Dr. Emanuel writes in response to widespread criticism of the Obama Administration during the International AIDS Conference in Vienna, as well as to the July 20<sup>th</sup> <em>New York Times</em> <a href="http://www.nytimes.com/2010/07/21/opinion/21tutu.html">op-ed by <strong>Archbishop Desmond Tutu</strong></a>, who writes:</p>

<p style="text-align: justify;">During my life, I’ve witnessed amazing advances in medical science. New treatments turn H.I.V. infection from a death sentence to a manageable illness. The cost of treating it is a small fraction of what it was 10 years ago. Meanwhile, more and more African nations have invested in the public health infrastructure needed to distribute AIDS drugs. I appreciate that tough financial times require the United States government to cut spending. But scaling back America’s financial commitments to AIDS programs could wipe away decades of progress in Africa.</p>
</blockquote>
<p><strong>Anand Reddi</strong> <a href="http://www.huffingtonpost.com/anand-reddi/truth-and-reconciliation_b_660586.html">responds on </a><em><a href="http://www.huffingtonpost.com/anand-reddi/truth-and-reconciliation_b_660586.html">Huffington Post</a></em> and specifically counters the argument that &#8220;we can&#8217;t treat our way out&#8221; of the epidemic:</p>

<p style="text-align: justify;">Results by Dr. Julio Montaner published in <a href="http://www.thelancet.com/journals/lancet/article/PIIS0140-6736(10)60936-1/abstract" target="_hplink"><em>The Lancet</em></a> demonstrated that provision of antiretroviral therapy in British Columbia halved the rate of new infections. <em>The Economist</em> <a href="http://www.economist.com/node/16635916" target="_hplink">writes</a> the study by Montaner is &#8220;the strongest confirmation yet that treatment and prevention are two sides of the same coin.&#8221;  Treating our way out is indeed possible.  In our recent <a href="http://journals.lww.com/aidsonline/Documents/Leeper%20and%20Reddi%20PAP.pdf" target="_hplink">article</a> in <em>AIDS</em>, Sarah Leeper and I demonstrated the unique dynamic-reciprocal relationship that HIV/AIDS, TB, and Malaria have on maternal and child health and vice-versa in the resource limited setting. I salute Dr. Emanuel for forwarding the dialogue on global health and advocating for comprehensive, integrated, cost-effective models of care that advance not just HIV/AIDS but other important health priorities including maternal and child health. Given that the provision of antiretroviral treatment can actually lead to prevention, eradication of new HIV infections is possible if the world community remains fully committed to funding cost-effective antiretroviral treatment initiatives.</p>

			<content:encoded><![CDATA[<p>A  new survey from the <a href="www.cdc.gov/">Centers for Disease Control and Prevention (CDC)</a> suggests that  heterosexuals living in impoverished communities are as much as five times more likely to be HIV-positive than the general U.S. population, regardless of race or ethnicity. In the US, the overall HIV prevalence rate for African Americans is eight times the rate for whites, and the rate for Latinos is three times the rate for whites. Yet in very-low income areas this CDC study found that these disparities do not exist. <a href="http://articles.latimes.com/2010/jul/20/science/la-sci-aids-poverty-20100720">The study</a> examined 9,000 people in 23 cities, finding that 2.1% of heterosexuals living in high-poverty urban areas were infected with the HIV virus, including 2.4% of those living below the poverty line and 1.2% of those living above it. This is in comparison to the 0.45% rate of HIV infection in the general US population. The authors hypothesize that the findings could account for many of the ethnic and racial disparities in HIV infections in this country, since African Americans are 4.5 times as likely and Latinos four times as likely as whites to live in poverty.</p>

Back home, two medical students examine the Obama Administration&#8217;s plan to let PEPFAR funding flatline while promoting a new Maternal and Child Health (MCH) initiative.  In a <a href="http://journals.lww.com/aidsonline/Documents/Leeper%20and%20Reddi%20PAP.pdf"><strong>commentary published in </strong></a><em><a href="http://journals.lww.com/aidsonline/Documents/Leeper%20and%20Reddi%20PAP.pdf"><strong>AIDS</strong></a></em>, Sarah C. 			<content:encoded><![CDATA[<p>This week at the<a href="http://www.aids2010.org/"> International AIDS Conference</a> in Vienna, Austria, the <a href="http://www.who.int/en/">World Health Organization</a> announced that <a href="http://www.who.int/mediacentre/news/releases/2010/hiv_treament_20100719/en/index.html">5.2 million people in low- and middle-income countries received antiretroviral drugs for their HIV infections in 2009</a>. This represents the largest increase in the number of people accessing treatment in a single year, with an additional 1.2 million people added to the four million who received antiretroviral therapy in 2008. While the increasing access to antiretrovirals is encouraging, over 10 million people infected with the HIV virus worldwide are still in need of therapy.</p>
<p>The WHO is calling for earlier treatment of those infected with the HIV virus, suggesting that if people are treated before their immune systems become weakened by the virus, HIV-related mortality can be reduced by 20% between 2010 and 2015. In addition, early therapy also has a benefit in preventing transmission of the virus. These new guidelines expand the number of people eligible for antiretroviral therapy from around 10 million to about 15 million people. The <a href="www.unaids.org/">Joint United Nations Programme on HIV/AIDS (UNAIDS)</a> estimates that the cost needed for HIV treatment in 2010 will be about US$ 9 billion.</p>

<p>For the first time in the 15 year-long search for an HIV prevention method that women can control, a vaginal microbicide gel called Viread has been shown to decrease the risk of HIV infection by as much as 54%.  Even though the microbicide does not prevent transmission in every woman who uses it, this is the first promising tool that women are able to use without the cooperation of the male partner.  This is an important consideration most of the new HIV infections in women living in Africa were acquired through forced sex with infected men who refuse to wear condoms. Women and girls represent 60 percent of the 22 million people infected with HIV living in Africa.</p>
<p><span id="more-1082"></span></p>
<p>The gel was developed by <a href="http://www.conrad.org/" target="_blank">Conrad</a>, a nonprofit organization based in Arlington, Virginia, and funded by the U.S. and South African governments under royalty-free license from Foster City, California-based <a href="http://www.gilead.com/corporate_overview" target="_blank">Gilead</a>, the world’s biggest maker of AIDS medicines. The Bill Gates Foundation also helped fund the trial.</p>
<p>In the clinical trial, coordinated by scientists at the Centre for the AIDS Program Research in South Africa (<a href="http://www.caprisa.org/joomla/" target="_blank">CAPRISA</a>), the use of the microbicide was compared to a placebo in a group totaling 889 women located in either the urban setting of Durban or the  rural setting of Pietermaritzburg in the <a href="http://uvtravel.co.za/index.cfm?Aid=1885888272" target="_blank">KwaZulu-Natal province </a>of <a title="KwaZulu-Natal province " href="http://marinesciencetoday.com/wp-content/uploads/2009/10/SouthAfrica-KwaZulu-Natal-Province.gif" target="_blank">South Africa.</a></p>
<p><strong> </strong>The main active ingredient of Viread was a 1% vaginal gel formulation of the nucleotide reverse transcriptase inhibitor <a href="http://www.ncbi.nlm.nih.gov/pubmedhealth/PMH0000198" target="_blank">tenofivir</a>. The pill form of tenofivir is already used in combination with other antiretroviral agents for the treatment of HIV and works  by slowing HIV’s spread through a patient’s body.  The secret to tenofovir’s success as a topical agent may be that it absorbs into the vaginal wall and into the cells targeted by HIV. Other gels have had to be sufficiently spread around and present in the vagina during intercourse in order to work.</p>
<p>Participants of the clinical trial were instructed to apply the gel 12 hours before and up to 12 hours after intercourse. Participants were told that the gel was experimental and were also counseled to use condoms or another means of HIV prevention. According to the trial results, when compared with the placebo group, women in the tenofovir group showed 39 percent fewer HIV infections. Within the tenofovir group, women who used the gel more than 80 percent of the time had 54 percent fewer infections than women who used the placebo gel .  For women who did not use the gel regulalrly ( less than 80 percent of the time) there was a 28 percent reduction.</p>
<p>Some theories as to why the gel did not perform better:</p>
<blockquote><p>The net impact seen in the study reflects the combined effect of many variables, only one of them the action of tenofovir, which penetrates into the vaginal tissue, protecting the cells that HIV targets for infection. Other variables include the prevalence of HIV infection in the male population; the number of sexual partners a woman had; the amount of AIDS virus (&#8220;viral load&#8221;) in an infected man&#8217;s semen; concurrent use of condoms; and, most important, the consistency with which a woman used the gel.</p>
<p>For that reason, the researchers said, it&#8217;s impossible to say how much protection this microbicide might afford any woman.</p>
<p>&#8220;We can only approximate it,&#8221; said Salim Abdool Karim of the University of KwaZulu-Natal in Durban, South Africa, who helped lead the study. &#8220;What you see is a mixture of the efficacy of the product mixed with the ability to use the product. It is fundamentally dependent on human behavior.&#8221;</p>
<p>Other scientists speculated that some women who became infected despite using the tenofovir gel might have been exposed to men with very high HIV load (which occurs soon after infection).</p>
<p>&#8220;My most likely explanation is that you have to go up on the dose,&#8221; said Anthony S. Fauci, who heads the National Institute of Allergy and Infectious Diseases at the National Institutes of Health. &#8220;You may have maxed out on 1 percent,&#8221; he said, meaning a more concentrated gel might produce greater protection.</p></blockquote>
<p>Over the past 15 years, six other microbicides were tested in 11 clinical trials, with none showing a protective effect.  <a href="../index.php/2009/12/14/latest-microbicide-to-fail-to-prevent-hiv-infection/" target="_blank">Previous gels</a> relied on drugs that weren’t specifically designed to target HIV.</p>
<p>Another important finding in the trial was that the gel prevented the transmission of genital herpes (caused by the herpes simplex virus-HSV) by 51%.  A genital herpes infection increases the susceptibility to HIV infection because the HSV virus can cause open lesions to form.</p>
<p>If further development and testing continues to be successful, more potent formulations combined with marketing that makes the product more appealing to women could lead to increased prevention of HIV transmission. <strong> </strong></p>
<p>A product could be ready as early as 2013 if the results are confirmed by a second study known as Voice that is enrolling patients.</p>
<p>Gilead, which donated the active ingredient in the gel, won’t participate in the commercialization of the product in developing nations. They are unsure of whether or not will market the gel in the U.S. and Europe. Conrad gave the rights to manufacture the gel to the government of South Africa to get the product to women in the country most affected by the disease as quickly as possible.</p>
<p>The findings of the study were described at the <a href="http://www.aids2010.org/" target="_blank">18th International AIDS Conference</a> in Vienna, Austria and the research article was <a href="http://www.sciencemag.org/cgi/rapidpdf/science.1193748v1.pdf" target="_blank">published in Science magazine</a>.</p>

			<content:encoded><![CDATA[<p>In an <a href="http://english.ruvr.ru/rtvideo/2010/06/09/video_9398937.html" target="_blank">anti-drug conference</a> held in Moscow recently, Russian President Dmitri Medvedev <a href="http://www.larouchepub.com/eiw/public/2010/2010_20-29/2010-24/pdf/18-20_3724.pdf" target="_blank">called for an a more globally unified effort</a> to end the drug trafficking of opium from Afghanistan and the social problems that are a direct result from its trafficking. With over 90% of the world&#8217;s opium originating from Afghanistan, President Medvedev believes that that current efforts by international organizations such as the United Nations, NATO and Shanghai Cooperation Organization, are not enough. Opium poppies are the raw material used to make heroin.   According to the U.N. Office on Drugs and Crime, heroin has created a market worth $65 billion and caters to 15 million addicts world-wide.</p>

<p>The effects of Afghanistan’s 375 ton per year opium and heroin export are also felt at home through direct use and passive exposure such as  <a href="http://www.state.gov/p/inl/rls/fs/140668.htm" target="_blank">second-hand and third-hand exposure</a>.  A new study that will be finalized this summer is expected to show that in Afghanistan 1.5 million people out of a total population of 30 million are addicts and that a quarter of those users are thought to be women and children.</p>

<p>Another study funded by the <a href="http://www.state.gov/p/inl/rls/fs/141848.htm" target="_blank">International Demand Reduction Program</a> sought to determine if there were unwilling addicts exposed to opium by evaluating the indoor environment of the homes of known opium users.  Air, surface, and hair samples from all the inhabitants living in the homes of the opium users tested positive for opium products.  Direct contact and hand-to-mouth transfer from items such as eating utensils, toys, pillows, and bedding may be a significant route of exposure to opium for children.  The long term consequences of this passive exposure on mental, physical and emotional development to the children are unknown.   We should not wait to see what these long-term effects will be and something should be done immediately to put an end to this abuse.</p>
<p>Another theory as to why and how early these addictions begin suggests that the easily accessible opium fills a void that results from lack of access to doctors and medicines. Opium is used to<a href="http://news.bbc.co.uk/2/hi/8687734.stm" target="_blank"> treat physical pains and ailments</a> as well as the <a href="http://www.nytimes.com/2009/05/06/world/asia/06kabul.html?_r=1" target="_blank">psychological pains</a> brought on by decades of war and poverty. One mother described how her children became addicted:</p>
<blockquote><p>&#8220;When my son was born, he had earache. We couldn&#8217;t get to a doctor, so I gave him opium to help him get rid of the pain. After my daughter was born, she got stomach aches, and I only had opium to give to her for medicine, so now they&#8217;re both addicted.&#8221;</p></blockquote>
<p>Other factors leading to increased use are the high unemployment rate throughout the country and the return of refugees from Iran and Pakistan who became addicts while abroad.</p>

<p>The <a href="http://www.washingtonpost.com/wp-dyn/content/article/2010/04/07/AR2010040704410_pf.html" target="_blank">disagreement</a> about how much <a href="http://www.forbes.com/2009/02/26/drug-trade-afghanistan-opinions-contributors_terrorism_mycoherbicides.html" target="_blank">criminal groups, insurgents and terrorists</a> are funded by the drug trade has made it more difficult to develop and follow a unified plan to end this problem.  I agree with President Medvedev’s sense of urgency, but how do we begin to put an end the destruction resulting from the opium trade?  Is it better to start from scratch or to work on coordinating the existing agencies that are evaluating the damage and setting up interventions?</p>
